1. Download Application Form (FILES INCLUDED: Application Form in PDF and JPG, Guideline file)
2. Print the Application/Membership Form (either PDF or JPG; Both file types are included in the download).
3. Fill out the form neatly and legibly in BLACK INK. Please avoid erasures. Make sure to complete all required fields, incomplete forms will not be processed.
4. Attach a 1×1 ID photo of yourself.
5. Snail mail to the address included in the Notepad file.
6. Wait for our confirmation e-mail to let you know that we have received your application.
